Cannabidiol, commonly known as CBD, is a phytochemical compound that is found in the cannabis Sativa plant.  It is a variation of the plant that we all know as marijuana, however, this variety is cultivated and known as Hemp. The Hemp plant is cultivated to produce high amounts of Cannabidiol (CBD) and low amounts of the psychoactive compound Tetra Hydro Cannabinol, commonly known as THC.

However, unlike THC, CBD is not psychoactive, nor will it get you high or cause you to fail a drug screening for THC.  This makes CBD a great option for those seeking relief from a variety of ailments without the mind-altering effects of cannabis or other side effects related to some over-the-counter and prescription drugs.  Many animal models suggest CBD may be effective for a wide range of issues stemming from inflammation, anxiety, pain, nausea, or insomnia.

How is it made?

CBD is extracted from the Hemp plant using various methods.  The extracted product is often referred to as “distillate” or “isolate” and is generally offered in “broad spectrum” or “full-spectrum”.  The 2018 Farm Bill, which legalized the cultivation, extraction, and use of industrial Hemp, allows up to .3% by weight of THC in the finished product.

Pain and CBD

There have been many animal model studies and some pre-clinical studies that leave researchers cautiously optimistic that CBD may be helpful in the treatment of some types of pain such as rheumatic diseases, fibromyalgia, pain associated with MS, back pain, and arthritis.  The effectiveness of CBD in many of these studies is improved in combination with THC.  Thus, if you are choosing CBD for any of these reasons and are not subject to drug screening, a Full Spectrum product, containing up to .3% THC would be most desirable.

Mental Health and CBD

In a 2019 study involving individuals with heroin use disorder, a double blind randomized placebo controlled trial found that participants who were giving the FDA approved drug EPIDIOLEX at 400mg and 800mg of CBD once per day had less cravings and response to drug cues vs the placebo group.  Further, the effects were still observed up to 7 days later, after a 3 day course of CBD.  The study suggests a potential role for CBD in relapse prevention of heroin use disorder.  Anxiety reduction was also noted in the individuals receiving the CBD.

In another study, indication shows that an approximate 300mg dose of CBD was the most effective at reducing anxiety during an anxiety-producing event, such as a simulated public speaking test.

Cancer and CBD

There were five studies that found some evidence that Sativex (an EU approved CBD/THC nasal spray) may help cancer related pain in some people.  Other studies investigating the effects of CBD on cancer -related nausea, pain, and decreased appetite have been mixed.  The symptom improvement is very individualistic and it is recommended that you consult your healthcare team as CBD may not be safe, appropriate or affective for all people and all cancers.

Your Heart Health and CBD

Some research has linked the use of CBD with several benefits for your heart including the ability to lower blood pressure.  Some of these studies indicate that CBD may be helpful for people with high blood pressure.  In one study, 26 healthy men were given 600 mg of CBD for 7 days and led to significant reductions in blood pressure in the short term compared to placebo.  However, the benefits were lost after 7 days of dosing due to tolerance build up.  However, increased blood pressure due to stress persisted.  There is also evidence from animal studies that CBD modulates the cardiovascular response to stress.  Further investigation is ongoing.

Conclusion

CBD is generally considered safe.  It can have some adverse side effects such as diarrhea and fatigue in some people   It may also interact with certain medications.  Often, medications that advise against eating grapefruit generally should also be avoided when using CBD.  Some of the benefits of CBD are noted as part of the entourage of cannabinoids found in the cannabis sativa plant, including some THC.  The benefits of CBD are quite individualistic and depend on many factors including quality, dosing, frequency and the presence of THC in the dose.
